Into the blending options of this new layer go to Drop Shadow and use the settings above.
Shut Down Icon (Photoshop drawing tutorial) - step 20
Now go to Inner Shadow and change the settings as in the image above.
Shut Down Icon (Photoshop drawing tutorial) - step 21
Finally go to Outer Glow and use these settings. Get out of blending options, resize this layer to fit your button and you're done!
